The cauda equina (latin for horses tail) is a group of nerves located in the lower back, so called because of their resemblance to a horses tail.
If pressure is placed on the cauda equina and cauda equina syndrome (CES) results it is vital that diagnosis and appropriate treatment is provided as swiftly as possible in order to reduce the risk of permanent debilitating injury.
Experts suggests up to 100 new cases of CES occur each year in the UK.
What causes cauda equina syndrome
There are a number of things that can be a catalyst for CES:
- Disc Herniation. Often at L4/L5 and L5/S1
- Injury/trauma
- Infections
- Tumour
- Spondylitis
- Following spinal manipulation

Red Flag Symptoms
It is often the case that the symptoms of CES develop very quickly, however it is possible for symptoms to slowly take hold.
Symptoms of CES are often described as ‘Red Flag Symptoms’. This is because a medical professional should be concerned when these symptoms present and take immediate action.
The symptoms include some or all these:
- Lower back pain. Often severe
- Bowel problems
- Bladder problems
- Saddle (bottom) numbness
- Pain down legs
- Poor control of legs and or feet
- Sexual dysfunction
Diagnosis and treatment of cauda equina syndrome
Accurate diagnosis and swift treatment is paramount for a positive prognosis.
Diagnosis is made with a medical professional paying close attention to the patient’s symptoms and confirmation is often made with the help of an MRI or CT scan.
Decompression surgery needs to take place as soon as possible once a diagnosis is reached, in order for the symptoms to be successfully treated. Delay in treatment may result in symptoms becoming set and permanent.
CES is rare but can be completely debilitating
CES is a rare condition with not many people having heard of it. However it is a concern of ours that CES is not as rare as many believe.
